Caesars Palace Online Casino's terms, at cl. 4, have the account holder authorize third parties, which may include a mobile operator, internet service provider, financial institution or government organizations, to help verify identity. FanDuel Casino's terms, at cl. 5.7, say bets can be placed only after the account is opened and identity has been verified.

In practice that means the name, date of birth and address on the form should match your Canadian identity document and bank records exactly.
